Intensive Care Coordination
ICC provides a single point of accountability to ensure that medically necessary services are accessed, coordinated, and delivered in a strength-based, individualized, family/ youth-driven, and ethnically, culturally, and linguistically relevant manner.
The ICC coordinator facilitates collaborative care planning and service coordination for youth with SED by promoting a collaborative relationship among the youth, his/her family and involved child-serving systems, through a Care Planning Team (CPT).
The team is comprised of both formal and natural family support persons. Together, they assist the family in identifying goals and developing an Individual Care Plan (ICP) and risk management/safety plan.
Eligibility: participant must receive multiple supports from providers, state systems or be enrolled in special education; needs care coordination due to multiple supports; caregivers must agree to voluntary participate in Care Coordination. Check with intake for exclusions.
